Cedars-Sinai awards seed grants to Clinical Scholar investigators so they can conduct studies to generate preliminary data to support an application for an NIH career development (K08 or K23) or research project (R01) grant. These one-year grants are intended to supplement other departmental funding available for the applicant’s research career development and should not be expected to be the sole support for the applicant’s salary and research efforts. Multiple grants of up to $30,000 are awarded, of which at least one will be an Eigler-Whiting-Mann Grant. Neil Eigler, MD, James Whiting, PhD, and Brian Mann established the grant that bears their names to recognize their longstanding commitment to Cedars-Sinai. The donors specifically wish to recognize the ongoing need to provide seed funding for promising postdoctoral biomedical investigators who have yet to establish a track record of competitive grant funding in a particular discipline. Awards are contingent on the availability of funds. Requirements for Application Applicants for Clinical Scholar and Eigler-Whiting-Mann grants must be enrolled in their first year of the Clinical Scholars Program and successfully completed the coursework and the grant writing workshops. Second-year scholars who did not receive a Clinical Scholar or Eigler-Whiting-Mann Grant the previous year may also apply. Successful applicants must be able to devote a minimum of 50% of their full-time effort to conducting research activities. The remaining effort may be devoted to clinical and teaching activities. The program is five years in length with four and a half years spent at Cedars-Sinai, including three months at Cedars-SInai Kerlan-Jobe Institute, and six months spent at Children's Hospital Los Angeles and Shriners Hospital for Children. The vast majority of the clinical rotations take place at Cedars-Sinai Medical Center allowing the residents to easily attend all conferences. The hospital provides superb on-call facilities, excellent staff and ancillary services including a magnet nursing program, a dedicated orthopaedic surgical floor, around the clock intravenous, phlebotomy and blood gas teams, EKG services, radiology technicians and patient transport services. In addition to a salaried position, residents receive several added benefits including a monthly housing and food allowances and free parking. The rotations are detailed below: PGY-1 Six months of General Surgery: Surgical Oncology, Vascular Surgery, General Surgery Night Float, Acute Care/ Trauma Surgery, Surgery Intensive Care, Plastics and Reconstructive Surgery Six months of Orthopaedic Surgery: Four Months of Orthopaedic Trauma three blocks), One Month of Hand, One Month of Musculoskeletal Radiology/Pediatric Orthopaedics) PGY-2 Two-month rotation blocks consisting of: Orthopaedic Trauma (three blocks), and one block each of Spine, Adult Reconstruction, and Hand Second year residents will also take in-house overnight call approximately every fourth night. We do not have a night float system. Post-call days are strictly enforced. PGY-3 Three-month rotation blocks consisting of: Sports Medicine (Cedars-Sinai Kerlan-Jobe Institute), Adult Reconstruction, Orthopaedic Trauma, Pediatrics (Children’s Hospital of Los Angeles) Third year residents will also take home operative call over the weekends approximately every two to three weeks. PGY-4 Two to Three Month Rotation blocks consisting of: Pediatrics (Shriners Hospital for Children Los Angeles), Foot and Ankle, Orthopaedic Oncology, Spine Surgery, and Hand and Upper Extremity Surgery. Fourth year residents will also take home operative/chief call approximately every eight days. The fellowship is open to residency graduates from any specialty who will be board-eligible or board certified prior to beginning fellowship training. Applications are accepted through ERAS. Upon completion, fellows will become eligible for board certification in Clinical Informatics. The fellowship program includes rotations across Cedars-Sinai Health System (CSHS) working with over 15 different clinician informaticists (including seven physicians board-certified in Clinical Informatics) across several specialties and practice settings. CSHS includes three acute care hospitals, one acute rehabilitation hospital and multiple diverse outpatient practice settings. CSHS features a diverse representation of practice models, including employed, private practice and faculty physicians. Rotations cover many areas across clinical informatics, including workflow analysis, design, and optimization, EHR implementation, change management, electronic health record (EHR) usability, system build, clinical decision support, quality and performance assessment and improvement, data analytics, research informatics, data security, interoperability, research, leadership, pharmacy informatics and pathology informatics. Didactic lectures will ensure exposure to all core content that may be covered on the Clinical Informatics board certification examination. To retain fellows’ clinical expertise and to promote familiarity with CSHS workflows, personnel and information systems, fellows will have protected time for clinical practice in their primary specialty. Fellows will also have the opportunity to work with startup companies participating in the Cedars-Sinai Healthcare Accelerator. the Heart Improving outcomes for heart disease patients was the focus of studies by Jae Hyung Cho, MD, PhD, and Lily K. Stern, MD, who in May won the 2021 Cedars-Sinai Clinical Fellows Award for Excellence in Research. "This award aims to foster clinical and translational research, enrich knowledge and encourage the development of our clinical fellows as investigators," said Mariko Ishimori, MD, assistant professor of Medicine, Committee Chair for the Clinical Fellows Research Award Programand associate director of the Cedars-Sinai site of the UCLA Clinical and Translational Science Institute. Jae Hyung Cho, MD, PhD She noted that despite the challenges posed by the COVID-19 pandemic, which led to cancellation of the annual award last year, many high-quality abstracts were submitted for review, from which four finalists were chosen. At the May 11 virtual event, the finalists presented their work to a panel of judges, who later announced the two winners. Cho's presentation focused on heart failure with preserved ejection fraction, which Cho said accounts for about half of diagnosed heart failure cases. More than 6 million U.S. adults have heart failure, which occurs when the heart cannot pump enough blood, according to the U.S. Centers For Disease Control and Prevention. Death rates are high for heart failure with preserved ejection fraction—just 25% survival after five years. There are no treatments available that increase life expectancy for people diagnosed with this type of heart failure, Cho said. Inflammation is thought to be behind disease progression, so Cho and fellow investigators treated laboratory rats with heart failure with preserved ejection fraction by using tiny particles called exosomes that were isolated from human cardiac progenitor cells, which regulate inflammation. The results showed that these exosomes: Preserved diastolic function Improved exercise capacity Reduced development of abnormal electrical signals in the heart Reduced systemic inflammation Decreased ventricular fibrosis, a buildup of inflexible fiber-like tissue in the heart's ventricular chamber Noting that exosomes are stable, durable and easy to store compared with cells, Cho called the results, if confirmed in humans, "very promising for next-generation therapeutics." Cho's mentors were Eduardo Marbán, MD, PhD, professor of Medicine, Mark Siegel Family Foundation Distinguished Chair and executive director of the Smidt Heart Institute; and Eugenio Cingolani, MD, assistant professor of Cardiology and director of Preclinical Research and the Cardiogenetics Program at the Smidt Heart Institute. Reflecting on how his work and outlook have been affected by the tumult of the past year, Cho said: "I am a physician-scientist and have always believed in science. But when COVID-19 appeared and our society was shut down, it made me question the power of our science. One year later, however, our fellow scientists developed vaccines, and we have the ability to control these tiny viruses. I am excited when I dream of carrying out research that will change the world for my children." Stern, a second-year cardiology fellow, presented her recent research on the impact of the 2018 revision of the United Network's donor heart allocation system. The allocation system is a protocol that determines who among many ill patients will first receive life-saving heart transplantation surgery. Lily K. Stern, MD The revised allocation system was intended to better prioritize transplantation of the sickest patients listed for heart transplantation and thereby reduce mortality while they wait for transplant. However, by transplanting the sickest patients on the wait list, there was concern there would be worse morbidity and mortality post-transplant. Under the mentorship of Michelle Kittleson, MD, PhD, professor of Medicine, Stern and fellow investigators performed a single-center retrospective study at Cedars-Sinai's high-volume heart transplant center involving 169 adult patients. They were divided into two groups: those who underwent first-time, single-organ heart transplantation under the old allocation system and those who underwent transplants after the system changed. Stern said analysis of the data showed that under the new donor allocation system, patients who underwent heart transplantation were higher risk with greater rates of mechanical circulatory support requirements, and they waited a shorter amount of time on the wait list prior to transplant. The study further confirmed that there was no significant difference in mortality between those transplanted in the year after the allocation system change as compared to those transplanted in the year before the allocation system change despite transplantation of a sicker population of patients. Stern said that the findings indicate that with careful patient selection, the revised allocation system may optimize wait list and post-transplant outcomes for patients needing heart transplant surgery. Stern also reflected on the past year, saying: "It was amazing to see researchers come together to rapidly chip away at important questions to provide guidance for clinicians on the front line. This year has given me a deeper appreciation for people who have and continue to devote their time to research, which is the glue that holds our clinical practice together. Coverage Analysis Cedars-Sinai requires that a coverage analysis (CA) is performed for any clinical research study that involves billable services (i.e., any clinical service, procedure or item that can generate a billing charge). The CA is a systematic review of clinical research study documents and determines whether the study is a qualifying clinical trial and which items, procedures and services are billable to Medicare and other third-party payers. The CA is conducted in compliance with the federal Clinical Trials Policy and is applied universally, regardless of the study’s funding source. For medical device studies, a CA determines whether the study device is billable based upon the U.S. Food and Drug Administration’s determination of the device’s regulatory status. For study devices conducted under an investigational device exemption (IDE), the CA ensures that the Centers for Medicare and Medicaid Services (CMS) has issued approval for billing of the investigational device and associated routine costs. The requirement for CA applies to all clinical research studies, regardless of expected inclusion or exclusion of Medicare beneficiaries and the source of funding. Every clinical research study is reviewed to determine whether it is exempt from a CA or should have a CA completed prior to Institutional Review Board submission. It's important to note that clinical trials for children are different than trials for adults. "Your child isn't the same as an adult—they're not even the same as other children. Infants react differently to interventions than their school-aged counterparts, and adolescents respond differently than those who are in puberty," says Dr. Irina Dralyuk, a pediatric pulmonologist in Cedars-Sinai's Maxine Dunitz Children's Health Center. But like clinical trials for adults, pediatric trials could offer children a chance to receive beneficial treatment—a new drug, therapy, device or procedure—before it becomes widely available. Clinical trials explained The goal of clinical trials is to advance scientific knowledge, but not all clinical trials are created equal. Some are noninvasive and require little involvement from study participants. Others test the effects of new therapies, vaccines and procedures and may call for multiple clinic visits, uncomfortable tests and treatments that produce side effects. All studies follow an established protocol that outlines what the trial aims to uncover, who is an ideal candidate, which treatments (if any) are being tested and how the results will be assessed. In randomized and controlled clinical trials performed in adults, researchers separate participants into two groups: an experimental group that receives the treatment under investigation and a control group that receives standard treatment or a placebo (water pill). A randomized, double-blind clinical trial, for example, randomly distributes study participants to receive either the placebo or the experimental treatment. In these trials, neither the patients nor the researchers know who received the treatment and who received the placebo until the study is complete. At the end of the trial, researchers analyze the results to determine whether the new treatment is effective. Researchers can't lump all children into one bucket—testing a treatment on a 17-year-old is not the same as testing a treatment on an infant. That's just one reason for the seven-year lag time (on average) between an adult approval and a pediatric approval for the same agent—that means adults have access to effective treatments years before they're available to children. "Principal investigators for pediatric trials provide focused care and attention to kids in these trials simply because children's bodies and minds are still developing," says Dr. David Ziring, associate director of the Pediatric Inflammatory Bowel Disease Program at Cedars-Sinai. So, while clinical trials for children are run in a similar manner as those for adults, there are a few key caveats: Informed consent: Depending on the age of the child, both the participating child and the child's parent need to consent to participate in the study. Older children and teenagers complete a separate informed consent. "We need to know they're willing to participate in the trial," Dr. Ziring says. Children's trials come last: Studies that are randomized and controlled set out to identify effective treatments for a particular condition. These trials occur only after preliminary research shows the agent is safe and effective in the lab, then in animals and finally in adults. "Kids are always the last to get tested with a new treatment," says Dr. Dralyuk. "By the time researchers begin trials for children, we know the intervention is safe and effective in adults." Placebo use: Clinical trials for children generally don't include placebos. "If there's evidence that a treatment works well in adults, it's not ethical to expose a child to a placebo and withhold a treatment that could improve their condition," says Dr. Ziring. Investigators who conduct clinical trials in children also pay particular attention to child development, including bone growth, hormonal changes, social and emotional development and other considerations that are specific to children. The first step for many parents is the government's repository of health trials at clinicaltrials.gov. "That's the most comprehensive source of information about clinical trials," says Dr. Ziring. If that process feels too overwhelming, talk to your child's doctor and ask about relevant trials at Cedars-Sinai and other major centers. Often, your child's physician is better equipped to identify appropriate trials. For children who have a specific condition, such as cystic fibrosis, leukemia or cerebral palsy, parents may be able to tap into national foundations and nonprofit organizations dedicated to the disease for information about relevant clinical trials. "If you're interested in clinical trials testing a specific drug, you can also do an internet search of the medication's name," Dr. Dralyuk says. "In many cases, pharmaceutical companies provide information or links to clinical trials investigating their treatments." Your child's participation could result in scientists discovering a more effective treatment for diseases that affect children all over the globe—and it could treat their disease more effectively than standard protocols. Still, there are risks involved with participation, too. In addition to potential side effects of experimental therapies, children may be subjected to the emotional and mental toll of frequent clinic visits, blood draws and assessments. "That's one reason these trials include incentives," Dr. Ziring says. "It's not to entice parents to enroll their child in the study, but rather to compensate them for their time and effort, and for the discomfort and inconvenience involved with participation." Before you decide to enroll your child in a clinical trial, make sure you get answers to three key questions: What are the potential risks and side effects of this protocol/therapy? How many visits will be required? Can you describe any invasive and/or potentially uncomfortable treatments so I can decide if the trial will be too taxing for my child? Then, if you really want to know the specifics, you can delve deeper with this set of questions: What tests, medicines and therapies will be part of the trial? Is this treatment approved for adults with similar conditions? Has this intervention been tested on other kids? How many clinic visits, blood draws and other procedures will be required throughout the course of the study? Will we receive reimbursements for participating in the trial? How does this protocol differ from my child's current regimen? Will my child need to be hospitalized? Who will cover the costs if my child suffers from complications during the trial? How long will the trial last? Who will support my child and me during the trial? Will we have access to the results of the trial? Will the trial include follow-up care? Learning as much as you can before enrolling your child in a trial offers the best odds of success. "When you review the informed consent with the physician who is running the trial, it's important to ask not only about the risks and benefits of participation, but also the risks and benefits of not participating—and what alternatives are available to your child," Dr. Ziring says. Remember, too, that every trial, invasive or not, is completely voluntary. You or your child can decide to withdraw from a clinical trial at any time, and for any reason. Your child's health—mental, emotional and physical—always comes first. Experiencing moments of anxiety, sadness, or fear is a normal part of a patient's cancer journey, but what about prolonged feelings of hopelessness, worthlessness, and thoughts of suicide? These more serious symptoms are all signs of depression, says Dr. Scott Irwin, psychiatrist and director of Supportive Care Services at Cedars-Sinai's Samuel Oschin Comprehensive Cancer Institute. "In the general population, about 7% of adults will experience an episode of major clinical depression," says Dr. Irwin. "That number increases to 15-25% for people with cancer." "People, and even providers, think 'of course they are depressed, they have cancer,' but it is not the norm and should not be ignored." Recognizing signs of clinical depression Symtoms of clinical depression include: Depressed mood, ongoing sadness Loss of interest or pleasure in usual activities Lowered energy or fatigue Change in weight or appetite Being agitated Increased or decreased sleep Feeling guilty, worthless, hopeless, or helpless Difficulty with concentration or indecisiveness Frequent thoughts of death or suicide Many of these symptoms can overlap with the side effects some patients experience from cancer treatment, such as lowered energy, decreased appetite, weight changes, insomnia, and impaired concentration. So how do you know? A good rule of thumb: If 5 or more of the symptoms of clinical depression listed above occur nearly every day for 2 weeks or more, it might be depression rather than side effects of treatment. Read: Blindsided by Breast Cancer: A Patient's Advice Dr. Irwin says the nonphysical symptoms are his key to spotting a cancer patient struggling with clinical depression. "When I'm assessing a patient, I focus on the nonphysical symptoms more," he says. "Feeling depressed, down, blue, or sad, an inability to experience joy, excessive guilt, and thoughts of actively ending one's own life are all signs that someone is dealing with clinical depression." "It's important that mental health is assessed so we can intervene, treat, and relieve the suffering associated with it," says Dr. Irwin. "In cancer, this is even more important because doing so will improve cancer care outcomes." In early 2012, Oraleen Johnson's ovarian cancer returned again. "You know the expression a cat has 9 lives?" says Oraleen. "After my 9th recurrence of ovarian cancer, I stopped counting." But this diagnosis was different. Oraleen, who was 70 at the time, was told by her oncologist in Las Vegas that after 22 years of treatment for ovarian cancer, there were no more therapies left to try. "After my 9th recurrence of ovarian cancer, I stopped counting." The news was shocking, but Oraleen's husband, Kent Johnson, wasn't ready to give up hope or say goodbye to his wife of 50 years. Neither were the couple's 3 children and 13 grandchildren. "We had everything to win and nothing to lose, so we began a search for hope and answers," says Kent. That search took them from state to state, where they visited one nationally acclaimed cancer center after another. Everywhere they went, they got the same answer: There were no treatment options left to try. The couple met with husband-and-wife team Dr. Alain Mita and Dr. Monica Mita, co-directors of the Experimental Therapeutics program. CPE Admissions Policy (PDF) Admission Requirements To qualify for admission to Level I and Level II ACPE Certified CPE programs, you must meet these requirements: Faith group identification and affiliation Completed and signed application form, including references, essays and $30 application fee Admission interview Acceptance into a CPE program and payment of the nonrefundable deposit Compliance with all health and background checks and requirements Payment of tuition fees during the first week of the CPE program Completion of orientation and self-registration with ACPE Signature of ACPE Appendix 5 Consent Form if supervised by an ACPE Certified Educator Candidate or Associate ACPE Certified Educator In addition to the above, these minimum criteria for each level must also be met. You'll learn how to listen to—and provide spiritual support to—patients and their families, as well as staff members, in their time of need. Our interdisciplinary approach combines observation, hands-on experience, mentoring and academic instruction. Courses Schedule Application Information Tuition Courses Cedars-Sinai's ACPE-certified CPE program is accredited to offer Level I, Level II and Certified Educator CPE. CPE units are offered throughout the year. Small student cohorts ensure you receive in-depth quality, hands-on chaplaincy training to effectively support patients, families and staff. Level I is a foundational program where students develop core clinical skills, professional identities and pastoral authority. Level II is for applicants who have already met Level I outcomes and is focused on professional spiritual care practice and competence. Certified Educator CPE is education for those who have successfully completed a minimum of four units of Level I/Level II CPE, and wish to become certified as ACPE educators. Level I and Level II CPE is offered throughout the year. Certified Educator CPE programs are not always available; please check for availability. One unit of CPE is at least 400 hours combining supervised clinical practice in ministry and no fewer than 100 hours of structured group and individual education. A half-unit of CPE is at least 240 hours combining supervised clinical practice in ministry and no fewer than 60 hours of structured group and individual education. Schedule CPE training is intense work, but Cedars-Sinai offers flexible options: Our full-time course is 40 hours per week for 11 weeks, plus on-call duty, and is held in the summer semester at Cedars-Sinai. Our 5-month extended courses are approximately 24 hours a week for 19 weeks, plus on-call duty, and are conducted during the fall-winter and winter-spring semesters. CPE group supervision takes place on Tuesdays from 8 a.m. to 5 p.m.; clinical hours take place at Cedars-Sinai and are flexible according to each student's schedule. Our 9-month extended courses, designed for students and working professionals, run over the course of the fall-spring semesters for approximately 37 weeks. 9-month extended units include 6 hours of CPE group supervision every other week (day of the week negotiated unit-by-unit). Students are responsible for 8 clinical hours per week which are flexible according to each student’s schedule. Students may complete all of their clinical hours at Cedars-Sinai or they can use an outside clinical site (i.e. congregation, etc.) to fulfill up to 4 of the 8 hours. The option to complete hours at another site is based on the site being a place where the CPE intern is already engaged in a pastoral capacity, such as a congregation where the student is already on staff in a pastoral care role. All outside clinical placements will need a Clinical Placement Agreement discussed and signed prior to the start of the unit. ACPE Certified CPE Unit Dates Availability 2022 Summer Full-Time June 7, 2022 – August 19, 2022 Closed 2022-2023 Fall-Winter (5-Month Extended) August 28, 2022 – January 6, 2023 Closed 2022-2023 Fall-Spring (9-Month Extended) August 28, 2022 – May 26, 2023 Closed 2023 Winter-Spring (5-month Extended) January 17, 2023 – May 25, 2023 Open 2023 Summer (Full-Time) June 5, 2023 – August 18, 2023 Open 2023 Fall-Winter (5-month Extended) August 27, 2023 – January 12, 2024 Open 2023 Fall-2024 Spring (9-month Extended) August 27, 2023 – May 24, 2024 Open Rolling Admissions: Applications are considered in the order they are received. All units, especially Summer units, fill quickly. Please apply 6 months to 1 year in advance of the unit start date. Note: Cedars-Sinai observes New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and Christmas Day. No classes or training are held on these holidays. Application Information On average, just 4–7 students are accepted to participate in each CPE cohort. Applications are processed in the order they are received, and all units—especially in the summer—fill quickly. We strongly recommend applying 6 months to 1 year in advance of your desired start date. Those training as professional chaplains—as well as for visiting clergy, religious leaders, seminary students and pastoral counselors—serve as students in a variety of settings: Hospitals, clinics and hospices Psychiatric and community care facilities Geriatric and rehabilitation centers Secular workplaces Congregational and parish-based settings What You ll Learn Through supervised encounters, CPE students learn to work with people who are vulnerable, in crisis and often in pain. This intense involvement, along with reflection on the interactions and feedback from peers and supervisors, enables you to develop new insight into yourself personally and professionally (pastoral identity). You'll also learn how to identify and address the needs of those to whom you offer spiritual care (pastoral competence). Using action and reflection (the clinical method of learning), you'll begin to articulate the meaning and purpose of your experiences as a spiritual caregiver and integrate these insights into your ongoing spiritual care practice. What to Expect Fully accredited by ACPE, Cedars-Sinai offers a comprehensive CPE program to qualified applicants without regard to race, culture, ethnicity, gender or gender identity, religious or spiritual beliefs, or sexual orientation. The program provides students with an opportunity to learn through a blend of: Providing spiritual care to people in crisis Reflection in seminars and workshops Case study presentations and reflections Group dynamic seminars Program Components The CPE Program at Cedars-Sinai is accredited by ACPE and combines structured teaching sessions and mentoring with hands-on clinical practice and spiritual reflection. You will be assigned to a peer group—which aims to have diversity in gender, race, culture and spiritual/religious tradition—and will be supervised individually and as a group. Approximately 70% of your time will be spent in clinical practice and 30% in educational activities, including instructional seminars with various faculty members, as well as clinical seminars based on student verbatims and case studies. In general, this is often due to differences regarding the norms to be used when making decisions about what is right, good, or fair in a specific circumstance. While not meant to be definitive or exclusive, below are some of the more common considerations, which prompt requests for ethics consultation: Concerns associated with end-of-life treatments (code status, advance directives, withholding/withdrawal of medical interventions, etc) Questions regarding surrogate decision-making Determining "appropriate" levels of intervention Patient refusal of treatment Real or perceived conflicts between and among patient, family, physicians, nurses, etc. Permissibility of using innovative or controversial treatments Moral distress of patient, family, or care providers Attending physicians, consultants and assigned nurses are invited to attend. Heralded as the best, if not the only way, to advance medicine, clinical trials are designed to evaluate new ways to prevent, detect, or treat disease. (1,2) Every medicine on the market, every pill we swallow, every vaccine in use, was once the subject of a clinical trial; countless lives have been saved by penicillin and the smallpox vaccine, to reach back in history and name just two. Current advances include a long list of immunotherapies now approved for use by the U.S. Food & Drug Administration (FDA) to treat lung cancer, as well as other cancers. The number of clinical trials being conducted in the United States and around the world is higher than it’s ever been. A total of 291,895 studies are currently registered with ClinicalTrials.gov, one of the largest databases of clinical trials in existence. Of these, 48,921 are actively recruiting participants. That’s up from just 1,255 trials, in total, when the National Institutes of Health (NIH) website first became available to the public in early 2000. (3,4) Yet despite efforts by both pharmaceutical companies and public health agencies to educate people about the potential benefits of clinical trials and to recruit volunteers, public awareness about clinical trials remains relatively low — as does participation, particularly among women, children, and minorities, who tend to be underrepresented in research. (5,6,7) A study published in May 2018 in the Journal of the American College of Cardiology found that while women were well-represented in trials of drugs for hypertension and atrial fibrillation, the numbers of those participating in studies involving heart failure, coronary artery disease, and acute coronary syndrome were significantly lower than the relative numbers of women affected by those conditions in the general population. (8) Cardiovascular disease continues to be the leading cause of death among women (and men) in the United States. Indeed, recruitment in general is considered a major challenge for those leading these trials, and many studies never get off the ground due to a lack of volunteers. Studies involving cancer therapies are known for being hard hit. Over 12,000 cancer clinical trials are in the recruitment stage, but estimates put rate of participation among cancer patients at as low as 3 percent of potential trial candidates. A recent report found that a significant number of trials fail to enroll more than a single patient. Among the most commonly cited reasons: simple lack of awareness among potential volunteers, mistrust of the process and fear of being a “guinea pig,” fear of not receiving the new therapy but rather the “standard of care” comparison therapy, restrictive inclusion criteria — for example, those who have had prior chemotherapy may be ineligible — and fear of side effects of new drugs. (9,10) Clinical Trials Past and Present The history of clinical trials is said to date back to 1747, when British physician James Lind conducted a systematic trial among British sailors with scurvy — a disease now known to be caused by a deficiency in vitamin C — by dosing some sailors on board a long sea voyage with a combination of orange and lemon juice to see if it helped cure the condition. A variety of studies soon followed, but it wasn’t until the twentieth century, when randomized trials came into play, that things took a big leap forward. (11) Randomization involves dividing participants into separate groups by chance, not by selecting them for certain features, to compare different treatments. The first recognized randomized trial was carried out in the mid-1940s by the British Medical Research Council to test the efficacy of the antibiotic streptomycin against tuberculosis. This trial also featured two other methodological advances in experimentation in that it was double-blind (neither investigators nor patients knew whether they were getting the active drug) and placebo-controlled (some patients received an inactive substance). Today, most large clinical trials involving drugs that are likely en route to FDA approval are randomized, double-blind, and placebo-controlled — a combination known in medicine as the gold standard. Still, trials involving medical devices and surgeries are frequently not randomized, and many, if not most, devices aren’t subject to the same clinical rigor as drugs. The vast majority of clinical trials involve drugs or biologics — medicinal preparations like vaccines and immunotherapies that stimulate the immune system to fight infection and disease. At last count, 133,007 such trials were registered with ClinicalTrials.gov, more than double the number of studies involving surgical procedures and medical devices combined. (4) Related: 7 Myths About Clinical Trials for Advanced Breast Cancer The Phases of Clinical Trials 1 2 3 and 4 Clinical trials are usually carried out in humans after laboratory and animal testing demonstrate that a treatment is promising, according to the NIH. The trials move through a series of phases and follow protocols including plans to balance potential risks with benefits. Trials are typically conducted in four phases: Phase 1 Researchers test a treatment in a small group of people (about 20 to 80), for a period of several months. The main purpose is to learn about safety and to identify side effects, often as dosage levels are increased. About 70 percent of drugs pass this phase of testing. Phase 2 Usually (but not always) a larger group of people (100 to 300) receive the treatment to further assess efficacy as well as safety and side effects. About one-third of experimental drugs successfully complete both phase 1 and phase 2. Phase 3 The treatment is given to still larger numbers of people (1,000 to 3,000) and can last several years. This phase is intended to provide investigators — and, ultimately, the FDA — with more complete information on effectiveness and the range of possible adverse reactions. Between 70 and 90 percent of drugs entering phase 3 studies succeed. After a successful Phase 3 trial, a pharmaceutical company can request FDA approval to market the drug or biologic. Phase 4 This phase, also referred to as post-marketing surveillance, occurs after a treatment has been approved by the FDA and is made available to the public. At this stage, researchers are supposed to track safety in the general population and gather information on benefits and optimal use. If problems occur, restrictions may be placed on the drug or result in its being taken off the market. (12,13) Should You Participate in a Clinical Trial People say they participate in clinical trials for a variety of reasons, including to play an active role in their healthcare; to help others by contributing to medical research; to gain access to treatments before they are widely available; to get medical attention and care they might not ordinarily receive; and even to earn money, according to CenterWatch. (14,15) Satisfaction rates among people who have participated in a clinical trial run high: A report published in 2013 by the Center for Information & Study on Clinical Research Participation (CISCRP) found that 95 percent of those who had taken part in a trial in the past said they would consider joining another in the future. (16) Still, many potential first-time volunteers never move forward. There are a number of reasons for that. Some, particularly minorities, are disturbed by memory of missteps in the past, primarily the Tuskegee Syphilis Experiment (1932–1972), in which African-American men were told by the U.S. Public Health Service that they were being treated (at no charge) for syphilis, but who in fact received no treatment at all for the communicable and often deadly disease. (17) Still others are cognizant of past trials that resulted in products or treatments that subsequently turned out to be problematic for some reason, such as thalidomide, the sedative marketed to pregnant women in the late 1950s for morning sickness that caused thousands of severe birth defects; and the Dalkon Shield, the intrauterine device (IUD) used in the early 1970s that prompted more than 200,000 lawsuits by women harmed by the device. Other commonly cited concerns include: fears about side effects; doubts that the therapy will be any better than the standard of care; and worry about receiving a placebo — as in, why would anyone want to risk getting a placebo, especially if they are very ill? (14) While experts in the field acknowledge that the risks of enrolling in a clinical trial are real and entail many unknowns, they also say that some concerns are unfounded. For example, placebos are rarely used in trials involving patients with more advanced or serious illness, says Kenneth Getz, founder of CISCRP, explaining that most use an active comparison drug, typically the FDA-approved standard of care. In addition, volunteers are free to withdraw from a trial at any time and for any reason. (18) Understanding Informed Consent Over the years, and often in response to mistakes from the past, practices have been put into place to protect and inform patients. For example, the Tuskegee tragedy led to a process known as informed consent, which involves informing prospective volunteers about the intervention in question and the potential risks involved. Informed consent is now considered both a critical component of patients’ rights and clinical trials themselves. (6) During the informed consent process, people should be provided with information that enables them to make an informed and educated decision about whether to enroll in the trial. That includes explaining to prospective subjects the purpose of the research, what their role would be and how the trial will work, and allowing them adequate time to ask questions and discuss things with family and friends. The FDA also notes that it’s important for people to understand their role as a “subject of research” — not as a patient. Finding a Trial That Suits Your Needs Clinical studies are conducted in a variety of types of locations, including hospitals, universities, doctors' offices, and community clinics. The location depends on who’s conducting the study. Funders can include governmental agencies, industry groups, individuals, universities, and other organizations, such as foundations and not-for-profits. Locations now exist in all 50 states and in more than 200 countries around the world, according to NIH. (4) Trials can be found for virtually every condition imaginable. While many of the most commonly publicized trials often involve high-stakes therapies, such as those for late-stage cancer, lower-stakes trials are plentiful. Registered trials include treatments for everything from acne to insomnia to anxiety. (4) And they’re not all about pharmaceuticals. Many clinical trials involve interventions with therapies such as botanicals, mineral supplements, and acupuncture — even aromatherapy and yoga for hot flashes, for example. (4) People with a preference for lifestyle interventions to prevent or treat illness can help play a role in moving those disciplines forward as well. A number of trials involving diet and exercise have been registered. These include studies assessing the effects of low-fat vegan versus Mediterranean diets on body weight and insulin sensitivity, run by the Physician’s Committee for Responsible Medicine, and the effects of intermittent fasting in subjects consuming a Mediterranean or Western diet, part of ongoing research at the Washington University School of Medicine on the prevention and treatment of age-related diseases. (4) In the end, whether to participate in a trial is a personal decision, but perhaps the most important takeaway is that the decision be an informed one.
