Community Builder Alice Coles 2005 Inspire Award Honoree -- AARP The Magazine Advocacy &nbsp; <h1>Alice Coles Community Builder</h1> <h2></h2> Inspire Awards 2005 Honorees<br /> <br /> Alice Coles<br /> Until 2003, most of the 114 residents of Bayview, Virginia, lived in the kind of abject poverty that is difficult to grasp: two- and three-room shacks with no running water and no heat, and the constant threat of fires from faulty electrical wiring. In the last year, most of those people have moved into modern housing, thanks largely to the efforts of Alice Coles, 53. <br /> <br /> In 1994, this single mother of two learned that the state of Virginia was planning to build a maximum-security prison on land that her ancestors had farmed for more than 300 years. &quot;We decided to fight it,&quot; Coles recalls, &quot;because we didn't have anywhere to go and we didn't have anything to lose.&quot; Coles, who at the time was making $5,000 a year handpicking the meat out of crabs, educated herself about how the Department of Corrections worked and traveled to Richmond to testify against building a prison less than a mile from where her children attended school. &quot;I didn't have two pennies to rub together, but I told them about our dedication to our land and our children. And they listened.&quot; <br /> <br /> Flush from that victory, Coles organized her neighbors and began applying for federal and state housing grants to turn the eyesore that was her community into a jewel. Today, 42 of a planned 132 homes are built, as are a greenhouse and a community technology center. &quot;We have to look within ourselves for that one gift God has given us and use it,&quot; Coles says. &quot;We just need to look around and see how we can make this world a better place to live.&quot; *The name of this award was originally the Impact Award. Community Mapping A Better Way to Make Disaster Plans Home Improvement &nbsp; <h1>Community Mapping A Better Way to Make Disaster Plans</h1> AARP Virgin Islands has joined with the Virgin Islands Emergency Management Agency (VITEMA), the Red Cross and several other government and civic organizations to begin a process that will help to uniquely identify each community’s or neighborhood’s needs and characteristics in the Virgin Islands. <br /> <br /> See Also: <br /> The new initiative focuses on both the physical and social characteristics of a particular geographic area to help disaster personnel access the area when a need arises. Additionally, by developing a strong relationship with leaders from each neighborhood, it is felt that disaster and recovery agencies will be able to keep their finger on the pulse of what is happening in each community in real time throughout the crisis.<br /> <br /> The plan is for these community leaders to work with VITEMA to help individual community members understand how emergency agencies work during a crisis. The project will run on a year round basis with special programs being brought into community centers where the residents will have immediate access to information about emergency procedures, sheltering, and other emergency related issues of importance to them.<br /> <br /> As the emergency personnel share information they will also have the opportunity to evaluate the unique needs or circumstances of each neighborhood. Those unique circumstances could include things like being located within a flood plain, having only one access road into an area, being prone to mud slides or even things like having a large concentration of elderly, disabled or young people who will be dependent on others for support during a crisis. <br /> <br /> “AARP hopes to learn which areas are most vulnerable from a “people” perspective, states Denyce Singleton, AARP VI State Director, “We want to know in advance who might need special assistance moving from their residence to a shelter.” <br /> <br /> By mapping out the island neighborhoods for their unique hazards, unique situations or populations, emergency personnel can make better educated decisions in preparation for an impending storm or better decisions about possible rescues after a disaster. <br /> <br /> “Community mapping is a mutually beneficial activity that will help emergency workers better estimate where and how to utilize precious resources before, during and after a disaster,” continued Singleton, “We also hope the project will help to foster closer community involvement with one another by getting people to think about disasters in a community perspective rather than a single family situation.”<br /> <br /> Another example of an issue being considered by the community mapping group is what safety measures to take in handling animals during a crisis. Whether the animal be the family dog, or cat or a herd of goats, sheep or horses, it’s important that each community’s situation be examined. By getting together to share information about the unique challenges experienced in your neighborhood, emergency services personnel can complete a needs assessment where issues can be considered and resolved before there is an actual emergency situation. <br /> If you would like representatives from the community mapping group to schedule a community event in your neighborhood, please give us a call. Try contacting a shelter, and share your walk with a furry new friend. Want to feel even better about your walking habit? Use those 10,000 steps as an opportunity not just to boost your health, but also to help others. If you’re ready to walk the talk about more often, here are some steps you might take: 1. Become a museum docent i.e., a knowledgeable volunteer who leads visitors through exhibits, interprets what’s on display and answers questions. If you can commit to several hours of instruction and an assignment that’s often longer than a year, you can put your presentational skills, curiosity and love of art, architecture or history to great use. Contact museums in your area about their docent programs. 2. Walk dogs at a rescue shelter. Pooches kept in kennels much of the day need exercise, socialization, play and love. If there’s a shelter near you, consider sharing your walks with a four-legged buddy. 3. Organize a “walking school bus.” Compared to our generation and that of our parents, very few schoolchildren today walk to school. Safety is a major concern. Launching a program that provides adult chaperones to kids along various pedestrian routes to school gives youngsters security, freedom, new friends, fresh air and exercise. It can also be a solution for towns that have reduced school bus service to save money. To set the wheels on the bus into motion, check out this plan. 4. Sign up for a . Register with the nonprofit organization of your choice, sign up your sponsors, recruit your friends, design or collect your T-shirt and lace up those shoes. Organizations that help folks with HIV/AIDS, food allergies, Alzheimer's disease, arthritis and others are making great strides with the help of volunteers like you. 5. Join a community cleanup. Your town probably organizes one for Earth Day in April, but what’s stopping you from taking disposable gloves and a small trash bag with you on your daily walk — helping to keep your neighborhood beautiful year-round? 6. Deliver meals to homebound seniors. Organizations that fight hunger need folks to prepare hot meals or fill bags with donated produce, meat and other foods. But once that’s done, many depend on volunteers to get it quickly to people with limited mobility. Check in with a food pantry or Meals on Wheels about working the midday meal run. Chances are you’ll also deliver a smile to someone who doesn’t get out often. 7. Welcome visitors and residents with a free neighborhood walking tour. More smaller communities are hyping their historical relevance as a way to support landmark preservation efforts, draw traffic to small-business districts, market the area and celebrate their unique heritage. Organize a theme, create a route, share what’s great about your town and meet new people. 8. Become a neighborhood greeter. Less formal than a tour guide, a volunteer greeter is a friendly resident whom visitors can call on informally to show them parks, shopping sweet spots, architecture and other hidden gems. Show off the neighborhood you’re happy to call home. 9. Share your sight with a fitness partner who is blind. Besides getting out for some exercise, blind neighbors also appreciate help with grocery shopping, reading and bill paying. 10. Volunteer as a walking buddy to an older person who lives alone. Elders near you who are isolated or frail can benefit from exercise and social contact with a regular walking partner. Photo: Lucille Sherman/Axios On a glorious October weekend, while the rest of Raleigh buzzed from an epic college football rivalry game, the state fair, artisan markets, pumpkin patch visits and kids&#x27; soccer games, Osprey Cove Drive was still. Just four days earlier, the street in northeast Raleigh&#x27;s Hedingham neighborhood became the site of when a opened fire, killing five and injuring several others. Police swarmed the area, tallied the dead and searched for the suspect for hours while waiting for the all clear. Details: Cop cars lingered at the end of a quiet Osprey Cove Drive Sunday morning. Entrances to Hedingham are decorated with crosses, dozens of bouquets and photos of the victims. Two trailheads on the Neuse River Greenway, where the gunman fled and shot several others, remain blocked off. The big picture: Elsewhere in Raleigh, the few lingering reminders of the shooting are the whispers of restaurant goers and a Raleigh Police Station memorial in honor of one of the five victims, an off-duty officer cop who was on his way to work when he died. The of the News &amp; Observer featured a black background and the names of the victims of the shooting. It asked the question many are asking: Why? Here&#x27;s what we know: The shooting left five dead, including a 16-year-old boy. The victims are 52-year-old Nicole Conners, 34-year-old Mary Marshall, 49-year-old Susan Karnatz, 29-year-old Raleigh Police officer Gabriel Torres, and 16-year-old James Roger Thompson.Two others were injured, including one woman who spent her 60th birthday at WakeMed in critical condition. Police have not provided an update on her condition. The suspect, a 15-year-old boy, remained at the hospital in critical condition as of Friday morning, Raleigh&#x27;s police chief said. Petitions to charge the suspect have been filed, Wake County District Attorney Lorrin Freeman said Friday. Freeman plans to try and move the case to superior court where he can be tried as an adult, she said. Sign Up Now As crazy as this sounded to many people, it was a success, and within a few years&nbsp;Cerreta&nbsp;was getting requests for advice from people in other states who wanted to start community cafes of their own.&nbsp;Cerreta eventually started a foundation called&nbsp;One World Everybody Eats&nbsp;(OWEE) and dedicated herself full-time to spreading the community cafe movement&nbsp;across the country. There are several differences between a community cafe and a regular restaurant. For starters, most community cafes are run as nonprofit organizations, which means they can afford to offer their food at lower prices. They generally rely heavily on volunteer labor, but if they have any paid staff, they make a point of paying&nbsp;them a living wage. One major difference for customers is the ability to choose portion size. At regular restaurants, portions have grown steadily larger over the past several decades. Since it isn&#8217;t always possible to take home your leftovers, this leaves many customers with the choice between wasting food or eating more food than they&nbsp;really need. While couples should have serious discussions about finances prior to the big day, they also need to be aware of how their property will be treated from a legal ownership standpoint once they tie the knot. Couples need to understand how states view property ownership &#8211; not only in the case of divorce, but also for individual liability purposes in the event that one of the spouses gets sued by a third party. <h2>How Is Marital Property Treated </h2> Marital property is treated as either community or separate property &#8211; and how it will be treated is dictated by state law. Whether you live in community property or separate property state depends on where you are domiciled. There are nine community property states; the remaining states are separate property states. In community property states, community property is generally shared equally between spouses, regardless of the source of the property. However, married partners in community property states can also have separate property &#8211; for example, if they owned it before entering into the marriage. <h2>Determining Your Domicile</h2> The term &#8220;domicile&#8221; has a very specific meaning for these purposes. Even if you have more than one home, you can only have one domicile. This is your permanent legal home &#8211; one that you intend to use for an indefinite or unlimited period of time, and the place to which you plan to return when you leave. Where your domicile is located really depends upon the circumstances. To determine where your domicile is, you must consider factors such as: Where you pay state income tax Where you vote The location of property you own or rent Your citizenship Your length of residence Business and social ties to the community The amount of time you spend in one place, however, is not always the determining factor.&nbsp;For example, say you and your spouse own a home or rent an apartment in Texas, have Texas driver&#8217;s licenses, maintain banking accounts in Texas, and list your Texas address on your federal tax return. However, you both regularly travel for work every week, with assignments all over the United States. Even though you spend the majority of the year away from Texas, you both consider Texas home &#8211; and taking into account all of the factors above, Texas is considered your domicile. <h2>Community Property</h2> Married couples (or some registered domestic partners in certain states) who live in Arizona, California, Idaho, Louisiana, Nevada, New Mexico, Texas, Washington, and Wisconsin live in community property states. The term &#8220;spouse&#8221; or &#8220;married&#8221; includes registered domestic partners in the states that allow for such designation (Nevada, Washington, and California). What Is Community Property? Property that you, your spouse, or both of you acquire during your marriage while you are domiciled in a community property state The part of the property that is bought with community funds (if part of it was bought with community funds, and part was bought with separate funds) Property that you and your spouse agree to convert from separate property to community property Any property that can&#8217;t be identified as being separate property What Is Community Income? Any income from community property Salary, wages, or other payment received by you or your spouse during your marriage Real estate that is treated as community property under the laws of the state where the property is located Income from separate property in some states, such as Texas, may also be counted as community income. For example, if stock ownership in a corporation is the separate property of one spouse, but pays quarterly dividends, the dividends would be counted as community income.&nbsp;Also, once a couple is married, debts incurred for the benefit of the marital home are considered community debt &#8211; although this can be tricky.&nbsp;If you have an issue, consult a tax advisor, financial advisor, or lawyer. <h2>Separate Property</h2> Unlike the nine community property states, married couples in separate property states typically retain individual ownership of assets acquired during marriage. 1. What Is Separate Property? But regardless of what type of state you&#8217;re domiciled in, it is important to identify which property is separate, and which property is community. Advancing Population Health Through acquisition and aggregation of data from multiple sources, along with sophisticated mapping technology, we have identified the neighborhoods in our catchment area where the most vulnerable populations live. Latinos, Asian-Americans (including Koreans and Filipinos), African Americans, and LGBTQ+ communities are among the high-risk groups that are currently underserved. As a result, we have launched a cancer outreach network and formed valuable partnerships with community leaders, organizations, legislators, healthcare providers, health educators, and cancer survivorship and advocacy groups to reach these populations. Our researchers investigate issues such as High rates of advanced breast cancer among women of Filipino and Korean descent Increasing incidence of liver cancer in the Latino community High risks of prostate, colorectal, and pancreatic cancer—and low rates of screening—among African American men Improving biomarkers for the early diagnosis of colorectal and esophageal cancers Increased incidence of HPV-related cancers and tobacco use in the LGBTQ+ community Elevated smoking rates among female Korean immigrants Increased melanoma risk among low-income Latinos and Non-Hispanic Whites Low rates of colorectal cancer screening among Korean-Americans Lack of HPV-associated cancer education and vaccination updates for teens and young adults from diverse racial/ethnic backgrounds Ways to increase physical activity in high-risk underserved populations Methods to engage underserved populations in clinical trials to increase access to potentially lifesaving cancer treatments Partnership Strategy A crucial part of our strategy is partnering with community organizations, such as churches, health clinics and youth groups, to provide cancer education and screenings. Through these partnerships, we are building relationships that create pathways for researchers to conduct community-based studies on the root causes of cancer disparities. The findings may enable us to identify the most effective interventions to reduce cancer risk, increase survival and improve quality of life in these communities. In an effort to disseminate cancer-related best practices regionally and state-wide, we have developed partnerships with universities such as Stanford and UCLA, and community organizations such as Susan G. Komen, Tower Foundation, and the California Dialogue on Cancer. Cancer Outreach Network In order to strengthen Cedars Sinai Cancer’s ability to develop and disseminate culturally appropriate, evidence-based cancer information that is tailored to the needs of underserved communities, we have launched our Cancer Outreach Network in diverse and underserved communities. Trained, bilingual community health educators and promotoras assess community needs and implement tailored cancer education workshops to disseminate information on cancer prevention and treatment, including precision medicine and genetics to address cancer disparities in local communities. Local data have helped guide the development of specific cancer screening events in neighborhoods around Los Angeles. Cancer Education Programs Diverse Communities in Faith Settings Our community health educators and promotoras lead educational seminars focused on specific cancer risks, screenings, treatments and clinical trial enrollment. This faith-based approach is a proven effective channel for delivery of health promotion programs, especially among racial/ethnic groups with high church attendance (i.e. Korean, Latino, Filipino, African American). The LGBTQ Communities of Los Angeles A network of culturally sensitive and trained, nurses and community health educators disseminate tailored cancer information at local LGBTQ+ Pride volunteer and provider events in Los Angeles (Long Beach, West Hollywood, Compton, Valley, Downtown LA, Whittier and San Gabriel Valley). LGBTQ+ people have a unique “cluster of risk factors” that lead to both greater cancer incidence and later-stage diagnosis. Our interventions are developed using a community-participatory approach in partnership with our LGBTQ+ Community Advisory Board. For more than three months, Caroline Carbajal made weekly house calls to one of her patients: a retired 76-year-old man who had been experiencing panic attacks and increased blood pressure. Her visits revealed that he was also straining to manage mounting bills and facing eviction from his L.A. apartment. Carbajal isn’t a doctor. She is a community health worker employed by Cedars-Sinai as part of its Community Connect Program. The effort is an innovative response to a growing body of research demonstrating that fundamental life challenges—such as struggles related to housing, food, finances, transportation, substance use or mental illness—have an enormous impact on an individual’s health. Identifying and addressing social determinants of health is the cornerstone of the Community Connect Program, which aims to screen every Cedars-Sinai patient for 13 social risk factors, says Katie Hren, LCSW, MPH, the program’s manager. In Embracing Our Community: Cedars-Sinai Connects Patients to Services for Homelessness When screenings uncover health-related social needs, social workers and nurse care coordinators consult an electronic referral platform powered by findhelp.org to match patients with support services. “It’s important for treatment teams to be aware of health-related social needs that patients are experiencing because these factors play a pivotal role in one’s health,” Hren says. “Armed with this information, caregivers can provide comprehensive, whole-person care.” Carbajal’s work is part of a Community Connect Program pilot initiative evaluating the effectiveness of assigning community health workers like herself to vulnerable patients to connect them with long-term assistance. Ultimately, Carbajal helped establish a relationship between her patient and two local nonprofits that provide ongoing food assistance as well as mental health and financial services. As a result, he avoided eviction and paid his bills; his panic attacks subsided and blood pressure stabilized. As a thank you, the man gave Carbajal a bow-accented houseplant. Outside the Los Angeles Christian Health Centers' Joshua House Clinic on downtown's Skid Row, the streets are lined with makeshift shelters pieced together from boxes, tarps, and blankets—daily reminders to clinic staff that the people they serve are among the city's most vulnerable. Los Angeles Christian Health Centers (LACHC) President and CEO Dr. Lisa Abdishoo has worked at Joshua House since she was a medical resident at Cedars-Sinai nearly 20 years ago. Confronting this community's complex health issues as part of her training inspired her to dedicate her career to homeless medicine. &quot;Patients know if they walk through our doors that we will take care of them.&quot; &quot;Once I saw how great the needs were, I thought, 'How can I do anything else? I have to do something about this,'&quot; says Dr. Abdishoo, who was hired as LACHC's medical director when she completed her residency. Community Clinic Initiative Dr. Abdishoo's passion for helping the underserved is shared by her colleagues at LACHC and the more than 30 other clinics participating in the Cedars-Sinai Community Clinic Initiative. The Community Clinic Initiative was launched in 2015 to strengthen LA's healthcare safety net by providing support and education to community clinics so the clinics are more equipped to face challenges like meeting growing demand; providing high-quality, efficient care with limited resources; and training the next generation of clinic leaders. The clinic's involvement will ultimately benefit the more than 10,000 patients LACHC cares for each year. &quot;Cedars-Sinai worked hard to understand the biggest challenges clinics face today—and designed an initiative that offers support where it is needed most,&quot; Dr. Abdishoo says. LACHC teams participated in yearlong programs focused on financial strength, quality improvement, and leadership development. Renewed focus on patients Dr. Abdishoo says her clinic's involvement will ultimately benefit the more than 10,000 patients LACHC cares for each year at its flagship clinic on Skid Row and at other sites serving Boyle Heights and South LA. As a result the Community Clinic Initiative, LACHC reevaluated floor plans by asking: &quot;How do we want the patient to feel?&quot; This led to the creation of more welcoming, easily navigable spaces and ways to reduce noise and stress. &quot;We want patients to feel dignified and respected.&quot; Bettina Lewis, chief operations officer, says it helped shift her focus from the bottom line to the patient experience. &quot;I put on a patient's hat to analyze each step they take from the time they enter the front door to the time they leave,&quot; she says. Yearlong programs focus on quality care, leadership development, financial strength and data analysis in clinics that represent the backbone of care for L.A.'s poor and uninsured. More than two-thirds of the community clinics in Cedars-Sinai's service area are participating in this initiative, designed to build capabilities that will enable them to more effectively respond to patients' complex needs over the long term in a changing healthcare landscape. Goal To significantly reduce disparities in health and healthcare for those most in need in Los Angeles by strengthening the leadership and effectiveness of community clinics. Leadership &amp Effectiveness Programs Managing to Leading This transformative leadership training program, developed in partnership with the Healthforce Center at the University of California, San Francisco, is for managers and supervisors earlier in their careers, specifically mid-level clinicians, administrators and other non-clinical staff who are managing or supervising others. Through seminars and coaching, as well as alumni networking, it provides participants with the knowledge, skills and confidence to effectively lead change and improve healthcare in today's complex and uncertain environment. Building a Culture of Quality This innovative offering teaches clinic teams how to best apply a range of quality improvement (QI) methodologies and embed QI at the organizational level. Facilitated by the Institute for High Quality Care, these individual program sessions are designed for multidisciplinary clinic teams with responsibilities for clinic management, project management, quality improvement and patient care. Through two types of offerings, (1) tactical, half-day learning labs and (2) a 6-part series for change agents, teams engage with coaches to focus on strategies to optimize ongoing improvement work and enhance QI engagement across the clinics. Advancing the Financial Strength of LA's Clinics This critical program aims to strengthen the effectiveness of clinics in these uncertain times. Designed for individual clinic staff and teams, including the CFO/COO/CEO, financial professionals, board members and CMO, the program includes general and targeted training both in person and online. Participants have access to benchmarking and tools, technical assistance and support to increase the effectiveness of the clinic's current and future sustainability models. This program is facilitated by Capital Link and offered by the Community Clinic Association of Los Angeles County. Future Program - Treating Addiction in the Primary Care Safety Net (TAPC) Designed for multidisciplinary health center teams interested in developing, implementing and strengthening medication-assisted treatment services, this innovative program provides support for community clinics in addressing patients with opioid addictions. This program is facilitated by the Center for Care Innovations, and incorporates both online and in-person sessions as well as mentorship, coaching and technical assistance. Future Program - Primary Care Psychiatry Fellowship This yearlong clinical fellowship provides psychiatric training for primary care-oriented providers, to allow for improved access for patients with behavioral health needs. Initially developed to fill a gap in primary care training, fellows of this program receive clinical education, training and mentorship to appropriately address behavioral health conditions commonly encountered in primary care settings. The fellowship is led by faculty from the University of California, Irvine, and the University of California, Davis, and culminates in a certificate of completion and continuing medical education credits for providers. Eligibility Unless otherwise noted, the following eligibility criteria are used for the Community Clinic Initiative. Clinic staff members are encouraged to participate in multiple programs, and more than one team or team member may be accepted. Applications are overseen by our grant partners. Location In keeping with our community health needs assessment, the initiative focuses on clinics in Service Planning Areas (SPAs) 4, 5 and 6, and targeted ZIP codes in SPA 8: Inglewood (90301, 90302, 90303, 90304) and Lennox (90304). Federally Qualified Health Center Community clinics should be Section 330 health centers or look-alikes, unless otherwise noted. Role Experience Each program has a unique focus and has been customized for different members of the clinic team. Medi-Cal and Medicare In addition to providing charity care, Cedars-Sinai pays the unreimbursed cost of caring for Medi-Cal and Medicare patients. These government programs set fee schedules that do not cover the actual cost of care. Cedars-Sinai is one of the largest providers of Medi-Cal services among non-government hospitals in California and serves more Medicare patients than any other hospital in the state. Despite the significant gap between actual costs and reimbursements, Cedars-Sinai provides the same level of highly specialized, advanced care to everyone. Cedars-Sinai Medical Residents Cedars-Sinai medical residents do regular rotations at community clinics across Los Angeles as part of their training in internal medicine and various specialty areas. While they gain valuable clinical experience treating a wide variety of health problems, the residents expand clinics' capacity to help the underserved. This community work inspires many to continue to serve vulnerable populations throughout their medical careers. $797 Million in 2017, Cedars-Sinai's financial contribution to community benefit totaled $797 million, providing community benefit through 5,500 targeted programs and activities and more than 200,000 health education and service encounters for the most underserved members of our community.
