Bacteria and viruses typically cause pneumonia. Fungi can also cause some types of pneumonia. Pneumonia affects millions of people around the world each year. In the United States, about 25 of every 10,000 people get pneumonia each year. That rate gets higher as age increases. Some people develop a serious case of pneumonia while in the hospital. But it’s more common to develop pneumonia outside a hospital setting. When this happens, doctors refer to it as community-acquired pneumonia (CAP). CAP is the most common type of pneumonia people experience. Although it’s not usually as serious as hospital-acquired pneumonia (HAP), it can still have serious complications. This article will look at CAP, how it compares with other types of pneumonia, and what you can do to protect yourself. <h3>Types of pneumonia</h3>Community-acquired pneumonia (CAP): CAP refers to pneumonia you develop outside a hospital setting.Hospital-acquired pneumonia (HAP): Also called nosocomial pneumonia, HAP occurs if you fall ill with pneumonia 48 hours or more after your admission to the hospital. HAP is usually more serious than CAP because it can involve bacteria that are resistant to antibiotics.Ventilator-associated pneumonia (VAP): A subgroup of people with HAP may develop VAP. It occurs in hospital patients who are receiving mechanical ventilation for respiratory failure. <h2>What causes community-acquired pneumonia </h2>Pneumonia has many causes. Research from 2017 references 26 common causes of CAP, mainly bacteria and viruses. Some fungi can also cause CAP. The most common causes of CAP in the United States are:human rhinovirus (common cold)influenza virus (flu)Streptococcus pneumoniae Other common causes include:the coronavirus SARS-CoV-2 (COVID-19)Haemophilus influenzaeLegionella bacteriaMycoplasma pneumoniahard-to-detect bacteria, such as Chlamydia pneumoniae So, how do humans come in contact with these germs? Some of them, like Streptococcus pneumoniae, already live in your nose, sinuses, or mouth. They can eventually spread to your lungs, where they cause disease. It’s unclear why they invade the lungs. You might also breathe in these germs if they are in the air around you. Certain activities or environments may increase your risk of coming in contact with them. <h2>Who is at risk of community-acquired pneumonia </h2> CAP and its complications are most likely to affect:adults older than age 65people with existing health conditionspeople with a weakened immune systempeople who smokepeople who are malnourished Children younger than 5 years also have a higher risk of CAP, as well as those who may interact with them in day care settings. <h3>Health conditions</h3> Certain conditions may increase your risk of developing CAP. According to a 2017 review of 29 studies, these include:chronic bronchitis or COPDasthmadiseases of the mouth, teeth, or gumsconditions that cause impaired function While the review was inconclusive on whether heart disease was a risk factor, other research from 2015 suggests it is. Other conditions, such as diabetes and liver disease, may affect your outlook with CAP. Researchers in the 2017 review also identified certain medications that may increase your risk:immunosuppressantsoral steroidsproton pump inhibitors and H2 blockers (both used to treat gastric reflux) <h3>Environmental exposure</h3> According to the World Health Organization, environmental factors may also affect your risk of CAP. These include:indoor air pollution from biomass fuelscrowded homessecondhand smokeliving on or near a farm with livestockliving near centers of industrial food animal production According to 2015 research, people who smoke cigarettes also have a higher risk of CAP. <h3>Zoonotic animal exposure</h3> Exposure to certain animals that may carry the germs that cause CAP could put you at risk. According to research from 2016 and 2017, examples include:birds, including poultryrabbitsgoatssheeppigscattlesick dogscats in labor <h3>Travel exposure</h3> The germs that cause CAP exist everywhere in the world. But some regions are more likely to have certain germs. Experts know that travel to large gatherings abroad contributes to the spread of germs that cause CAP. Traveling, spending time on a cruise ship, or staying in a hotel can increase your risk of exposure to these germs. If you have symptoms of pneumonia, be sure to tell a doctor of any recent travel. It may help them make a diagnosis. <h2>What are the symptoms of community-acquired pneumonia </h2> Pneumonia is a serious, sometimes fatal disease. The main symptoms of CAP include:coughproduction of sputumfeverdifficulty breathingheadachechest painmuscle achesirritability and restlessness (in infants)confusion (in older adults) In some people, breathing issues and fluid buildup in the lungs may become severe. This may lead to respiratory failure and the need for oxygen therapy or mechanical ventilation. <h2>How do doctors diagnose community-acquired pneumonia </h2> If a doctor notices you have symptoms of CAP, they will likely perform or order the following:Medical history: A doctor will ask about your medical history to better understand how you may have become ill.Physical exam: A doctor will perform a physical exam to check for fever and lung sounds consistent with pneumonia.Chest X-ray: A chest X-ray can show any fluid buildup or inflammation in the lungs.CT scan: A doctor might use a CT scan to confirm a diagnosis or rule out other conditions. CT scans may be more effective at detecting pneumonia than chest X-rays, but they are more expensive and take longer.Complete blood count (CBC): A CBC can help doctors see whether your immune system is fighting the infection.Electrolyte panel: An electrolyte panel can check your electrolyte levels as well as your kidney and liver function.Blood, sputum, or urine tests: Blood and sputum cultures, as well as urine antigen tests, may identify the exact cause of the infection, influencing treatment.Molecular testing: This is the standard way to test for viral pneumonia, such as cases caused by COVID-19. Research from 2020 has also shown that molecular tests, like polymerase chain reaction (PCR) tests, may also be better than cultures at detecting bacterial pneumonia. A doctor will also consider other potential diagnoses. Some illnesses with symptoms similar to CAP include:acute bronchitis or other respiratory infectionchronic obstructive pulmonary disease (COPD)heart failure or heart attackpulmonary embolism (blood clot in your lung)tuberculosis <h2>What is the treatment for community-acquired pneumonia </h2> If you have CAP, your treatment plan will depend on your symptoms and the specific cause of your infection. A doctor will also consider how severe your symptoms are. According to 2017 research, 80% of people in the United States can treat CAP at home. If your doctor determines you have a milder case of CAP from bacteria, they may prescribe antibiotics for you to take at home. A doctor will likely prescribe one of the following:amoxicillinazithromycinclarithromycindoxycycline It’s important to take antibiotics as prescribed. If you don’t follow the proper regimen, the bacteria could become antibiotic resistant. If you have a more severe case of CAP, you may need to get treatment in a hospital. Healthcare professionals can provide a variety of intravenous (IV) antibiotic treatments based on your specific situation at a hospital. But antibiotics do not work for cases of viral pneumonia. You may need to let the virus run its course. In some cases, you may be able to use antiviral medication. Examples include:remdesivir (Veklury) for COVID-19oseltamivir (Tamiflu) for influenzainhaled ribavirin for respiratory syncytial virus (RSV) There are other actions you can take to manage symptoms of viral CAP:Take over-the-counter pain relievers, like acetaminophen or ibuprofen, to reduce fever.Drink plenty of fluids to stay hydrated.Get enough rest to allow your body to recover.Quit smoking if you regularly smoke. If your symptoms are severe, you may need to go to the hospital. Additional treatments in the hospital may include:oxygen therapyintravenous (IV) fluidsrespiratory support with a ventilator <h2>What are the potential complications of community-acquired pneumonia </h2> With CAP comes the risk of possible complications. These are more likely if doctors don’t make a timely diagnosis or if initial treatments don’t work. Possible complications include:Pleural effusion: Pleural effusion is the buildup of fluid in your pleura, the space between your lungs and chest wall.Empyema: Empyema is the buildup of pus in the pleural space.Lung abscesses: An abscess is a pus-filled cavity that, in this case, forms in your lung.Sepsis: Sepsis is an extreme immune response to an infection.Acute respiratory distress syndrome (ARDS): ARDS occurs when the lungs become severely inflamed and fill with fluid, preventing oxygen from getting into the blood. Complications like sepsis and ARDS can cause organ failure and death. <h2>What is the outlook for people with community-acquired pneumonia </h2> With rapid and appropriate treatment, many people fully recover from CAP without complications. Young people tend to recover fully more quickly. If CAP is due to bacteria, you may start to feel better within 5 to 7 days of starting antibiotics. Still, it can take months for all your symptoms to resolve. If you have a mild case of CAP, full recovery is possible with rest, antibiotics or antivirals, and sleep. More severe cases of CAP may require lengthy hospital stays to help keep you stable and aid recovery. For people admitted to intensive care, the death rate can be as high as 23%, according to 2021 research. <h2>How can I prevent community-acquired pneumonia </h2> Taking precautions against illness, such as getting an annual flu shot, can reduce your risk of CAP. The Centers for Disease Control and Prevention (CDC) recommends that people ages 65 and older get a high dose flu vaccine. People over 65 can also consider getting the pneumococcal vaccine to prevent CAP. This is especially important if you have other health conditions or if you smoke. You may need a booster shot if you received your vaccine before age 65, or if you have a weakened immune system. Other everyday actions you can do to help prevent disease include the following:Wash your hands frequently, especially after using the bathroom or being in public.Cough into your elbow when you need to cough.Keep a physical distance from people who are ill.If you smoke or vape, consider quitting to reduce damage to your lungs. Such damage can make you more vulnerable to pneumonia infection. <h2>Takeaway</h2> Community-acquired pneumonia (CAP) is the most common type of pneumonia. After much hedging from series creator Dan Harmon, the movie adaptation of the beloved NBC series is officially a go. Peacock has ordered the film, snapping it up out of what it says was a highly competitive situation. Fans have been demanding a movie followup since the fan-led social media movement #SixSeasonsAndAMovie launched in 2014. Original stars Joel McHale, Danny Pudi, Alison Brie, Gillian Jacobs, Jim Rash and Ken Jeong will reunite with Joel McHale as an executive producer; Chevy Chase, Yvette Nicole Brown, and Donald Glover are not listed among the movie&#8217;s cast. When IndieWire asked about their apparent lack of involvement in the project, a Peacock spokesperson responded: &#8220;These are the only details we are releasing at this time.&#8221; McHale tagged Glover among his other cast mates when he posted the below tease on Twitter the morning of the movie announcement. Chase, who is also on Twitter, was not tagged. The &#8220;Community&#8221; movie will come from original series creator Dan Harmon, executive producer Andrew Guest, and from studios Sony Pictures Television and Universal Television, a division of Universal Studio Group. Peacock is also acquiring the full series library (non-exclusively). Per the official press release: From the TV series that predicted its own movie, comes the least predictable movie of a TV series that referenced a lot of movies and TV. "The fan that Instagrams every day about 'Community,' how can you tell them, 'Yes, it's definitely going to happen, but it may be between one and eight years from now' - which is how the industry works, especially when you factor in pandemics and whatnot," Harmon said recently when last hinting at the possibility of a movie. "It just feels like psychological torture, from a fan's perspective, to keep looking into the backseat of a station wagon and saying, 'Who wants McDonald's?' And to just keep driving down the freeway. I don't want to be guilty of causing that sensation. It just takes so long sometimes." Casey published 7 October 2022 The "six seasons" will finally get "and a movie" with the Community movie (Image credit: NBC.com) The Community movie is actually happening, folks, so grab your bandolier of paintball ammo, shout "pop, pop!" and (at some point) turn on Peacock. That's because the beloved NBC (later on Yahoo!Screen) sitcom Community is coming back.&nbsp;And, not to get your hopes up, but Dan Harmon just gave us a reason to be very excited about the movie's cast (see below). And, thankfully, the Community movie is bringing more than a fistful of familiar faces. That said, some original cast names are glaring by their omission. And, well, then there's Pierce. We don't expect Community to raise the dead (or fix the ties with Chevy Chase). But, yes, Community, creator Dan Harmon's biggest success story before we met Rick and Morty, is finally getting the film that many have clamored for for years. It's no shock, as we're in the age of streaming, where all of the best streaming services keep needing new content. So, let's break down everything we know about the Community movie, and what we hope it offers. <h2>Community movie cast and actors Which Greendale alum are coming back </h2> (Image credit: NBC.com) Variety (opens in new tab), which broke the news of the Community movie, answered some wishes (and created some new ones) for the movie. It said the following cast members are returning for the movie:Joel McHale as Jeff Winger, the disbarred lawyer who matriculated at Greendale to get a degree and wound up getting friends tooDanny Pudi as Abed Nadir, aspiring film-makerAlison Brie as Annie Edison, the dedicated student trying to shed her high school nickname of "Little Annie Adderall"Gillian Jacobs as Britta Perry, who's always ready to protest, even if she just found out about the causeJim Rash as Dean Craig Pelton, who had more costume changes than Britney Spears at the VMAsKen Jeong as Ben Chang, who tried to make 'Changesia' happen On social media, Joel McHale (opens in new tab) and other stars celebrated the announcement, and he tagged unannounced cast members Donald Glover and Yvette Nicole Brown, whose returns are not confirmed or even hinted at. Cameos, at this stage, seem more likely. Glover, who barely uses his Twitter account, did not respond.&nbsp; Speaking to Variety (opens in new tab), Dan Harmon gave reason to hope for Troy and Abed in a mooovie, saying "I think that Donald is coming, based on word of mouth, but it's just the deal isn't official or wasn't official. It would be difficult to really commit to doing this thing without Donald... So I believe he is coming back. I think if there's names missing from a list, it's because the names that are on the list, their deals are agreed upon enough that it's OK to say they are on the list, and anybody that's not on a list, it's just not the case yet. So there is nothing official about anybody being out." Brown (opens in new tab), on the other hand, quote-tweeted Joel McHale post, writing "It's happening! Congrats, #Community fans! You guys did this! &#10084;&#65039; #SixSeasonAndAMovie" Joel McHale seemingly accidentally cast Gillian Anderson in the film, with a tweet where he tagged the X-Files star instead of Gillian Jacobs. Anderson (opens in new tab) tweeted "Gillian Jacobs who? I'm in @joelmchale &#128521;," so now we need her as one of Britta's relatives. Chevy Chase will probably not come back as Pierce Hawthorne, as the character is dead and Chase's very public ousting from the series is still fresh in all of our heads.&nbsp; But speaking of Pierce we have two other dream casting wishes. First of all, we need Giancarlo Esposito back as Gilbert Lawson, Pierce's half-brother. Our second ask? It would be fantastic if Esposito and fellow Breaking Bad/Better Call Saul alum Jonathan Banks reunited in the Community movie. We'll explain why in the section below. <h2>Community movie crew Who else is returning</h2> Dan Harmon, of course, is also coming back. The show's creator will serve as executive producer and writer, with writing support from Andrew Guest (30 Rock, Community). Executive producers include McHale, and returning Community EP's Russ Krasnoff and Gary Foster.&nbsp; <h2>Community movie release window</h2> (Image credit: Neil Jacobs/NBC) Unfortunately, this is where things get even weirder. While we want to tell you a Community movie could hit Peacock next year, or in 2024 or 2025, there is no evidence to that. All there is, in fact, is one quote from Dan Harmon, from August, before the movie was confirmed. "I will now say, [about the Community movie], it's a matter of when." Harmon told Newsweek (opens in new tab), "It may be between one and eight years from now, which is how the industry works, especially when you factor in pandemics and whatnot." But since Community ended in 2015, seven years ago, what's another eight years? <h3>Community movie What does six seasons and a movie refer to </h3> Back in season 2 episode 21, "Paradigms of Human Memory," a clip show made up of new clips, Abed kept telling Jeff he believed in the success of NBC series The Cape. And that it would get "six seasons and a movie." Jeff didn't believe him. And ever since then, "six seasons and a movie" became the rallying cry of Human Beings (Greendale's mascot) everywhere. Not only because Community slowly lurched its way to that sixth season on Yahoo!Screen, but because the film was still on everyone's wish list. <h2>Community movie plot What we want to see</h2> Dean Pelton summoning the study group back to Greendale - either for a 10 year reunion or something else - makes the most sense. That's why we don't think Harmon and Co. will do something that straightforward. Instead, we have a far-fetched idea that's very meta. Which gives us hope that Harmon may be thinking of something similar. If the Community movie could bring back Giancarlo Esposito and Jonathan Banks, we'd love for Banks to not play criminology professor Buzz Hickey (aka Pat Nichols). Instead, imagine if Banks was re-cast as another secret long-lost relative of Pierce's. The gang could even call him 'Good Pierce.' You'd do all of this because Greendale Community College needs funding to survive, and the gang decided that Hawthorne Wipes could provide that funding. Community loved to mock symbiotic relationships and so on, but we imagine that Hawthorne Wipes may not be the treasure trove it once was, as 'Good Pierce' would probably give all the workers raises to a livable wage, decent benefits and time off. In an incident thought to have been sparked by a seven-year-old boy pressing the button on a police call box, one of two plainclothes police officers tries to arrest a girl, thought to be 14, who was repeatedly insisting she had done nothing wrong. Before the heights of Avengers: Infinity War and Endgame, the Russo Brothers had a huge part to play in developing acclaimed sitcom Community. Among fans of the show, the phrase “six seasons and a movie” has been vital over the years, as a Community movie has been anticipated since the show’s end in 2015. Now, it seems the project is picking up the pace, as stars of the show have offered promising updates. <h2>Alison Brie Joel McHale and more confirm Community movie conversations </h2> Since Community ended in 2015, the stars of the show have gone onto incredible heights. That much is certainly true of Donald Glover aka Childish Gambino, who has found immense success as a musician. Getting the cast together one last time amid their schedules has proved difficult, but Alison Brie has been one of the first cast members to offer a recent update on the project.
